Community Health Worker

We know that many returning citizens have faced barriers and distrust in the healthcare system—often entering incarceration with unmet health needs, and rarely finding support or healing inside. Our certified Community Health Workers (CHWs) are here to change that story. With a trauma-informed approach and lived experience, our CHWs walk alongside participants as trusted guides, helping them navigate health care, mental health services, and access to healthier food and wellness resources.

Our approach is personal, holistic, and rooted in understanding: we meet people where they are, listen without judgment, and build trust one relationship at a time. By connecting members to primary care, mental health support, substance use treatment, and nutrition resources, we help address the root causes of health inequity and empower individuals to take charge of their well-being. With trauma-informed advocacy, education, and ongoing support, our CHWs open doors to a healthier, more hopeful future—one built on trust, respect, and genuine partnership.
